La intersexualidad causa rechazo e infertilidad.
Los expertos sugieren que la detección temprana de esta alteración con el tamizaje neonatal contribuye a mejorar la calidad de vida. Al no contar la persona con órganos genitales completos se reduce su probabilidad de tener hijos.
‘Victoria’ (nombre protegido) es alta y lleva suelto su largo cabello de color negro y rizado. Su mirada guarda cierta timidez, pero también una tristeza que la ha acompañado toda su vida. Sus ojos se humedecen y su voz se entrecorta al relatar lo mucho que cambió su vida al enterarse de que era intersexual.
‘Victoria’ dejó de ser la pequeña inquieta de su casa para convertirse en una joven poco sociable, de escasa sonrisa, y optar por no mirarse al espejo ni salir por miedo de que la descubran.
Recuerda que cuando tenía 15 años su rostro era poco femenino; la voz gruesa y el no desarrollo de sus mamas preocupó a su madre, quien la llevó a un especialista. Cuenta que ahí fue cuando un doctor la chequeó y le explicó sin sutileza lo que estaba pasando con su cuerpo. “Me tildó de varón, pero yo me sentía mujer y me han criado como tal. Por un tiempo no quise saber nada de doctores hasta hace seis años que volví a consultar a una cirujana. Ella me explicó que mis cromosomas eran 46 XY (personas con cromosomas típicamente masculinos que presentan en sus genitales externos diferentes grados de feminización) y que mi clítoris estaba agrandado, pero en mis genitales externos poseía una vagina uniforme y gónadas masculinas (testículos) internas”, dice.
Hoy, a sus 26 años, ‘Victoria’ quiere operarse. El mes pasado, con ayuda de Silueta X, pidió al Ministerio de Salud que costee la cirugía, que costaría alrededor de $ 3.000 en un centro privado.
Francisco Vera, endocrinólogo y presidente del Colegio de Médicos de Guayas, explica que intersexual es una persona que nace con los dos sexos (tiene ovotestis, ovario y testículos) y existe una ambivalencia en el sexo de la persona. Asimismo, indica que en esta anomalía, que es genética, se da una alteración cromosómica que involucra a la endocrinología.
Vera asegura que a los 12 años los cambios hormonales se manifiestan, por lo que es la edad apropiada para someterse a una cirugía para corregir la apariencia de los genitales de acuerdo al género que se desee, “pero ahora hay que ser cautelosos por las nuevas leyes del Código Penal”. Agrega que cuando los pacientes pasan los 20 años se trata con hormonas femeninas o masculinas para desarrollar la personalidad que se necesita.
A diferencia de Vera, el cirujano urólogo Efraín Guillén considera que lo ideal es que se intervenga antes de que el menor vaya a la escuela. Asegura que además de la identidad de género una de las mayores afecciones que presentan los intersexuales es la alteración en la fertilidad. “Si es un chico de 23 años y ha tenido los testículos arriba, están atrofiados porque cuando no se encuentran en el sitio correcto sufren una hipertrofia o atrofia. En las mujeres los úteros son infantiles, no llegan a desarrollarse adecuadamente, por lo que se presentan problemas de infertilidad”.
El pediatra del Hospital de Especialidades Abel Gilbert Pontón, Francisco Obando, señala que para la detección precoz de los casos es importante el tamizaje neonatal, solo así se determinaría el tipo de intersexualidad.
Explica que existen cuatro tipos de intersexualidad: cuando los genitales internos son masculinos y los externos son femeninos; poseer todo el sistema femenino, pero externamente manifestarse una estructura genital masculina; cuando hay una mezcla de los dos tipos de intersexualidad; y el más complejo cuando no existen manifestaciones externas. “Este último es un problema hormonal porque tiene genitales femeninos, pero se comporta con algunas estructuras pequeñas no tan visibles a la inspección del médico pediatra. Son casos difíciles de detectar”.
Agrega que de darse cualquiera de estos casos se procede a hacer exámenes hormonales, de aspecto biológico y psicológico para determinar la operación que se necesita.
Según Obando, la primera reacción de los padres cuando se les explica que sus hijos nacieron intersexuales “es el rechazo, penosamente, y la falta de comprensión. En el país sigue siendo un tabú este tema; las personas lo ocultan y no permiten detectarlos. Los casos visibles son muy pocos”.
Es el caso de ‘Victoria’; por miedo al rechazo de su familia la única que lo sabe es su madre. “Ella es la única mujer de mis cuatro hijos. Siempre soñé con tener una hija. Me duele cuando los amigos o la familia se burlan de su apariencia”. Precisamente esta situación es la que aspira a cambiar con la operación, que según asegura, no ha podido realizarse por falta de dinero.
